Online Disclosure Acknowledgement

In order to process your application, Federal Law requires that we ask you to receive your required loan disclosures in an electronic format. These disclosures can be printed out while you are online. We will also include a copy of them in your loan application package that we will mail to you upon the completion of your online loan application.

Avidia Bank Customer Identification Program

As a result of the September 11 terrorist attacks, Congress enacted the U.S.A PATRIOT ACT of 2001. In order to make it more difficult for terrorists and other criminals to use the countrys financial system for illicit purposes, the law requires financial institutions to establish procedures to obtain, verify, and record information that identifies each person who opens and account.

What this means to you: When you open a deposit account, apply for a loan, or otherwise establish a formal banking relationship with Avidia Bank, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your drivers license or other identifying documents.
